The North Carolina Healthy Opportunities Pilot (HOP) is a state and federally funded program to help improve the overall health of qualifying Medicaid members and improve healthcare costs. We offer food, transportation, housing and interpersonal safety support services.
To potentially qualify for HOP services, you must be in a select Medicaid program, live in one of the pilot counties (Bladen, Brunswick, Columbus, New Hanover, Onslow, Pender), have qualifying health need and a food, transportation, housing and/or safety need.
