BURGAW – The Pender County Health Department (PCHD) is once again partnering with Pender County Schools to offer the annual influenza vaccine to students in grades K-12 and school staff. This convenient opportunity allows students to receive their flu shot during the school day, with parental consent.
To participate, a completed consent form must be submitted by Friday, Oct. 18. Parents and staff have the option to fill out either a paper consent form or complete the process digitally online. Paper consent forms can be requested through your child’s school.
Flu vaccines will not be administered without a completed consent form. For those choosing the online option, links to the digital consent forms are provided below, along with vaccine information sheets:
