PCS high schools utilize StudentCentral to collect and manage eligibility forms electronically. A help sheet is located below to assist with account creation and form completion. All forms must be completed in StudentCentral before an athlete participates in tryouts, practice, or contests.

Middle School Forms

All forms must be completed in StudentCentral before an athlete participates in tryouts, practice, or contests. Once Student Central accounts are created, hard copies of forms may be submitted to the school athletic director.

Insurance Forms

PCS Board Policy 3621 requires insurance coverage to participate in athletics. Private insurance and Medicaid meet this requirement. Coverage may be purchased from your desired insurance company or through k12studentinsurance.com. Insurance Enrollment Flyer
